Headquartered in New York, PATIENT TRANSIT (logistics system) was established by a group of medical offices, long term care and post-acute care stakeholders in 2011 to improve the quality, patient safety, cost, and satisfaction aspects of transferring patients between acute, post-acute, and ambulatory care organizations in the greater New York metropolitan region. Based on input from various clinicians and funding provided during initial phases, the PATIENT TRANSIT platform was designed to include the most pertinent information during a resident’s admissions and transfers. The initial pilot of the PATIENT TRANSIT mobile logistics system consisted of four nursing homes and four hospitals which included New York Presbyterian Hospital, Hebrew Home for the Aged at Riverdale, Bon Secours Health System – Schervier, Isabella Geriatric Center, and Parker Jewish Institute. This successful pilot has led to the rapid expansion of the original capabilities and allowed PATIENT TRANSIT to emerge as a leader in improved care transitions.
